How do I report a safety incident and keep the OSHA log?

Under Incidents, tap Report an incident and pick what happened: an injury or illness, a near miss, property damage, or a safety observation. Say when and where, add a photo, name who was hurt and who saw it — someone in Veristep or a typed name and company — and, for an injury, what it was and how far the treatment went. The report is frozen once it is sent; a correction is added underneath with a note saying why. The owner's people and the subcontractors on the job never see it, and nobody but the firm's administrator and its named safety person sees the injury itself.

The supervisor or project manager then works it: records the investigation and its root cause, raises follow-ups with an owner and a due date, or a deficiency where something on site has to be fixed, and closes the incident once every one of them is done. An injury closes only after it has been investigated and every injured employee has a decision on the OSHA log. An incident can be withdrawn with a reason, unless it is already on the log.

The administrator or the safety person decides, for each injured employee, whether it goes on the OSHA log and with what outcome — a death, days away, a job transfer or restriction, or another recordable injury — and keeps the days away and restricted up to date, to 180. For a privacy entry the log carries no name; the name is kept on a separate confidential list. Set up your establishments and which jobs belong to each under Incidents → Setup; the 300 log, the 300A summary (hours come from the timesheets, with any adjustment noted, and the injury rates per 200,000 hours are worked out) and each 301 print in the layout of OSHA's forms, and the 300A is certified with your name and title. A death must be reported to OSHA within 8 hours, and an in-patient hospital stay, an amputation or the loss of an eye within 24 hours — the Incidents page shows the deadline in red until the call is recorded.
